Our departure point is that in healthcare the products/services will have to survive in a unique ecosystem with unique challenges and unique opportunities. Excellent outline of some of these challenges and opportunities can be found in this Harvard Business Review article by Clayton Christensen, Richard Bohmer, and John Kenagy.

Therefore, while the basic principles for startups, customers and investors apply (and here’s a fresh example), there are additional considerations that are very important for healthcare startups.

As we are preparing our first HealthStartup event on October 5, we receive quite a lot of questions on how we plan to evaluate the participating projects – and how we plan to measure the output of the event.

Our goal is to develop a tool that would give buyers of the startups’ services – as well as potential investors – a simple, straightforward indication of the project’s impact and viability. It should also be useful as a guideline for the startups themselves in evaluating potential markets, raising funds, and communicating with various target audiences.

With that in mind, we’ve started developing a “project scorecard” which will include three aspects of an innovation and produce an overall score indicating the viability of a project.

The scoring on a simple numeric scale (1-4) will be carried out by participants during the event; scores would be tabulated and communicated after the event.

Attached is our initial thinking on the parameters within the scorecard. We would very much appreciate your input, any additions (or reductions) to the list of the question, clarifications, etc.

Will we get the healthcare ecosystem to collaborate on improving mankind?

We have very high development cycles of devices and techniques (18 months or less) AND an industry moving at a ludicrous speed.

We have an enormous amount of incompatible data, coming from:

different devices,
via different systems,
via different (mobile) OSs

Despite great efforts of organisations like Continua Health Alliance, it's an illusion to think that -someday- we will be using the same (open) techniques/standards worldwide to collect and combine health-data.

Striving towards compatibility and interoperability is essential, but I believe this should be complemented by focussing on efficient ways to aggregate all that incompatible data (the equivalent of Friendfeed) and display it as a coherent stream in a personal health record.

Fusion with data from energy-sources, city- & home-services and social media will further leverage the potential of the collected data.

Extra advantage is that the collected data can be searched upon anonymously. Possibilities to improve mankind by developing cures & remedies faster, improving diagnoses, improving adherence, ... and at the same time lowering the (healthcare)costs and giving more opportunity for 'face time'.

Summary of notes of the AAL workshop in Brussels on June 7, 2011

Personal summary

Many platforms and many applications and many users = many end-to-end products/services = 'The Long Tail' in assisted living. Going from 'one size fits all' to 'one size fits one' (quote from openurc).

Open source, flexibility, synergy and interoperability are the way to cope with current and future society variables and the velocity of technological evolution. Committee not fond of small scale projects (like Flanders Care) or closed source (like Apple).

User based involvement and consolidation of players in the ecosystem is imperative.

Whereas I think building data-aggregations (function like translators) will offer solutions for agile connectedness, none of the participants I spoke to seem to agree. Also my believe that creating an open environment through regulated APIs would leverage the potential of SMEs, didn't have many supporters.

AAL products and services have the advantage they aren't (always) considered as medical devices.

Matchmaking in AAL: find information/products/projects/... I can't understand why the EU hasn't started with making a list of people and companies involved, there's no public searchable list or inventory! If a complex ecosystem is one of the barriers, you should start by identifying the stakeholders!

Preparing for Westartup healthcare, I dug up a simple scorecard I used to determine the impact of an ICT - innovation in healthcare.

The scorecard gives a score, based on questions from 5 different angles:

Care organisation

What's the ROI? (workflow, efficiency and general cost reduction)

Does it have a quantifiable impact on marketing and recruitment?

Will it simplify making business/management decisions?

Is it interoperable with other aspects of the healthcare ecosystem?

What are the costs to train people to work with the product/service? How simple is it to use?

Care consumer

Does it offer convenience? Will it allow to reshape healthcare to fit people's lives (and not the other way around)?

What about participating in the process? Will it help the consumer to interact?

Exactly how does it improve the quality of the treatment from a consumers perspective?

Will it offer peace of mind?

Is it easy/simple to use?

Care practitioner

Can you quantify the improvements on efficiency, workflow and time?

Is it user friendly? Is it simple to use? Is it intuitive?

Will it enhance interaction with other practitioners?

How interoperable is it with the other tools I use?

Can it simplify decision making?

The weight of the 5 scores might differ for each product/service, but calculating an overall result is feasible and meaningful.

Cloud storage for the exchange of health information

For several years I'm complaining that paper health records are getting lost when one of the elderly residents of our carehome is hospitalised.

We did all kinds of efforts to prevent loss of the papers, but no success till now.

In case of loss we had to fax (alas, no email) the papers to the central reception of the hospital (alas, no fax at the specific department).

I was approached by a hospital and a software vendor to find a solution.

A logical suggestion was to use cloud storage (like Amazon, Dropbox, Nomadesk, ...) and to use encrypted folders (per user or per organisation) for such papers. Such cloud solutions are easy to use, very flexible, easy to implement and safe enough for this purpose. When hospital staff reacts outraged 'how unsafe this is', I point them to the current 'solution' where we fax the papers to the central reception.

I'm very curious which cloud solution they are going to produce, how cheap it will be and how user friendly it will be.

Questions for the future of home automation in health care

Next thursday I will be member of a panel (together with Joachim De Vos from Living Tomorrow & Philip Minnaert from Care Home Carpentier) discussing about the future of home automation in health care.

Is home automation (or technology in general) for elderly "comfort"or "necessity"? (responding to attractiveness, progress, HR deficit)

And it doesn't look like a medical (or other stigmatizing) device. Acceptance is guaranteed because such a shiny device is a 'wannahave'.

I wonder how long it will take before you can use Facetime on it.
With a GPS inside you will 'warned' when a friend is near, when you should get medicine from a nearby pharmacy, which subway to take, ...
You could communicate with relatives, doctors, ...
Notifications will appear when to take your medicine, when to go to the doctor, ...

And the real winner would be to integrate different monitoring solutions.

Should consider the device as a medical Toughbook, but vastly improved and much smaller.

Recently I was looking at 2 mhealth business plans, in particular the parts about sensor design.

The sensors should be wearable to guarantee permanent monitoring.

What struck me was that both companies were looking at "patient acceptance" as a HUGE hurdle for sensor sales.

Looking at the design of their devices, I understood their problem better: "They are UGLY!!!".

The devices had that typical "designed by a medical professional from the former USSR" signature.

If you want to solve patient acceptance, let Louis Vuitton or WESC take care of your design.

The burden of being sick and the necessity to be monitored can be worsened by making you wear ugly or uncool devices, especially with children.

Parents want their sick kids to grow up like 'normal' children.

If you can give these children a sensor that is cool to wear, this coolness might even 'lighten' the situation a bit for that child.

All types of wearable, unobtrusive sensors will have a great future because of simplicity and ease of mind.

Nick Hunn already gave fine examples like doorknobs and toothbrushes for monitoring, but why don't we also use helmets, clothing, wigs, earphones, scarfs, seats, ...?

I'm convinced that if you trow in more designers, this will make the sickness easier to wear (ha!).

Haven't read about the love that people feel about their device(s) or "second self". Now i'm even more convinced that upcoming baby boomers will embrace mhealth (broad definition).

Tying together the healthdata that will come through a giant firehose will indeed be very challenging. Companies that will specialize in health data integration and aggregation (slide 25 & 26: http://www.slideshare.net/BartCollet/mhealth-is-huge) will prosper.

Now i'll have a look at some examples/companies that Kent posted and i'm not familiar with ... yet.

I attended a meeting the other day where buzzwords like “mhealth”, ehealth, “connected health,” “wireless health”, and a slew of others (collectively “xhealth”) were thrown around the room like confetti at a parade.

At one point, the discussion became pretty heated around the attributes of the mhealth market compared to ehealth as well as which was higher in the perceived health information technology (HIT) hierarchy. I listened carefully as each participant passionately shared their unique point of view about the size and definition of each respective market and, of course, how their company was positioned to be the market leader. I have to admit, it was pretty tempting to jump in especially when someone in the group tried to create a new buzzword in an effort to plant their flag in yet another nebulous and ill-defined mobile healthcare niche.

In the race for healthcare innovation, coining buzzwords, and carving up prospective market segments, it seems like people have lost sight of the real prize — adoption. Specifically, near-term adoption.

Freemium in care: Zorgbeheer care-platform looking for webdev

Zorgbeheer (dutch for 'care management') is a platform for Flemish care-managers since 2006.
It grew from a small private communication platform for carehome owners to a communication platform for the captains of care.
The website has about 5 to 600 unique visitors a day.
There's a strong demand for further development, which should take the platform from it's current situation (stage 1) to a perpetual stage 3.

Stage 1

Mostly inspection, supervision and small improvements of the current situation.

Stage 2

Major overhaul with open source CMS (Joomla, Drupal, ...) or own development.
Improvements in matchmaking
Offering mobile content

Stage 3

Crowdsourcing care professionals for needs and building web and mobile apps to fill those needs ... and offering the apps for free.

Where's the money?
First two stages will be paid by commercial revenues (ads).
During the last (perpetual) stage, the webdev company can develop apps for individual care organisations too. The webdev business card being a solid (and proven) reputation as Zorgbeheer-platform-builder.

Why in English? It's a site in Dutch!
As far as I know, there are no platforms offering the stage 3 solution; improving care by offering web and mobile tools for free.
This format might be interesting for international development.

Requirements

Good knowledge of Joomla CMS (in stage 1)
Proficiency in mobile development
Experience with mapping, communication and community (matchmaking) tools
Determined to produce apps that are Fisher Price easy.

6) The importance of ‘voice’: i fully agree. But don’t underestimate the imortance of interface design (simplicity – i like the term ‘fisher price design’)

Baby boomers do NOT want to go to a senior house,
the cost to house them in a senior house are unsustainable anyway.
And if the cost would be sustainable, there wouldn't be (human) staff to take care of customers.
There's only one way to cope with these problems and that's at home monitoring, collaborative (family) caring and preventive medicine. Mhealth can play a major role in these 3 solutions.

Improve patient - doctor communication with a smartphone

Dutch insurer VGZ made this great iPhone app (Android coming soon) to improve communication between patient and doctor.

After accepting the User Agreement you can use this app to:

prepare your visit to the doctor by taking notes (you can use a FAQ for your specific medical problem)
use these notes (checklist) during the visit
you can even record (by doctors consent) the doctors diagnose

All notes and recorded material are for personal use only and can't be published publicly.

Other features include finding your doctor by GPS and/or rating the service by certain criteria.

I really like the FAQ-feature where you can have a look at questions that most people -with a similar medical problem- ask their doctors.

Beta for one week for crowdsourcing and bugtracking ... and then releasing it. Talking about fastdevelopment!
And the Posterous Android app, which i'm using to post this, is also extremly easy to use. Big WOW is the easyness to post accross different Posterous sites.

A quick update on the development of our public bulletin board.
The Archos 7 was a complete failure as you might have read.

I then choose an Acer Aspire Z All-in-one touchscreen PC.
Equipped it with Opera in kiosk mode.
Took some time to set everything up (and remove truckloads of bloathware), but the entire system is now tailor-fit to our needs.

Using the posterous menubar I'm adding more and more content:

briefing overview
workschedules
contacts

During the first weeks it was discouraging to see employees hesitating to use the digital bulletin board.
Even more discouraging to hear employees stating that "I prefer paper" (although I incorporated printable reports with RunJasperReports).
But as we keep adding information via the menubar ... more and more employees are using the board, more and more frequent.

Mission accomplished.

Looking for mobile systems where physicians can subscribe and authorize medication.

When physicians visit patients at their home (or carehome / care-center / serviceflat), they should be able to (via network or WiFi):

Lookup patient (/EHR)
Lookup patient medication
(eventually add medication)
Prescribe & Authorize medication
(eventually add preferred pharmacy)

All via their mobile phone, completely paperless

The patient can then visit the pharmacy and receive the medication.

My online search and my extensive Evernote 'library' didn't give me any answers.
Can you give me a hint?

mHealth SWOT-analysis: blame it on speeding

Is mHealth a hype or not?

That depends on which mHealth segment you are talking about. The mHealth-definition is ready to be re-written due to it's rapid development (like the health2.0-definition).

There are differences between wireless communication in healthcare, tracking systems, mobile apps, at home monitoring systems, ... all of which are considered "mhealth".

Some segments have tremendous future on a short term, some on a longer term, some will be buried by legal barriers, some will have zero chance due to government incompetence and ignorance.

And then you have consumers that are only aware of the shiny side of technology and are outraged the possibilities aren't rolled out more rapidly. Such an outrage, together with social media, fuels hyping.

The main threat for mHealth (broad definition) stays it's recent acceleration in development AND the fact that it's now available at reasonable prices. Going at warpspeed the mHealth development takes some corners too fast. Mhealth comes at such speed that panicking competitors can only shoot it before asking questions and governments doing likewise. Together with these consequences, this leads to even more blurring and confusing definition(s).

Better definitions will lead to better understanding and correct estimates of the real potential of the different mHealth segments.

We've always had an electronic bulletin board.
It did consist of a simple form on our intranet. When filled in, the data was stored in a MySQL database and emailed internally to the Gmail inboxes of the management (8 people).

Follow up of the messages was simple: reply to the message and the management is informed.

Challenge
A lot of that shared electric bulletin board information should be shared to more employees than only management, but also unavailable to the general public!

And if targeted to all employees, that board should be extremely easy to consult and read.

Rapid and fast reading + useability are key.
And reactions of the management (follow-up) on information topics should be open to all too.

Here we go!
First tests with yammer, present.ly, .. didn't run very well because users found it too difficult to use (comparison: here).
And there was a lot of confusion on what info should be put on email and what on Yammer. People had to open different apps and the character limit was to low.
Main usergroup are nurses and while they are incredibly gifted people, most of them are not very interested in using electronic systems ... especially if they consider it slightly cumbersome.

My task is to find a system that is not only supereasy, but also perceived as supereasy.

Less = more
Keeping this simple I chose only to use Gmail as a communication platform.
Public communications (post + reaction) is posted by email to a private posterous, which can be consulted by all employees via a touchscreen tablet.

Management only has to open their inbox to post, read and react. All other employees can consult (only consult, not react) the posterous bulletin board.
The posterous website is lightweight, responds very quickly and it's layout can be trimmed down to a very basic, Spartan looking, company bulletin board.

For the management I've also set up Gmail filters which makes it easier to flip rapidly through all messages.

Cost = 0,00 EUR

Tablet choice
We only need a touchscreen tablet with Wi-Fi and a decent browser (and/or RSS-reader).

An iPad was considered overkill as we only needed very limited functionality.

And because we work with Google Apps, choosing for Android was a more logical step.

After looking at the specs of different contestants, I bought an Archos 7 Android touchscreen for 150,00EUR.

Tought I had everything figured out, but ...

Scrolling is too cumbersome on this Archos 7.

Even when disabling all extra features, consulting the bulletin board is'n as snappy as i wished for. Sorry, but 2 too 3 seconds is too much 'waiting' time.

And this Archos has it's own Android appstore. Needless to say this store doesn't offer other browsers (which might be faster).

Other setback is .pdf files, posterous shows them with Flash. And Flash will only be supported from Android 2.2.

I got what I paid for and the system is currently unavailable too be used as an internal bulletinboard for all employees.

Maybe I have to go for the iPad or an Archos101.

Questions for the mhealth market for seniors

Too bad I couldn't attend the senior mhealth market 2010 conference.
If I had gone, I would have searched for answers on following questions.
Questions:

Mobile OS for elderly (for smartphone + touchscreen). I personally think such OS doesn't have a great future, but i get a lot of questions for the need of such OS. Do such OS'es exist?
Apps for elderly that are 'triple blonde proof' (although i prefer the term "Fisher-Price"-apps). Which companies are good at this?
Mobile phones conceived as hub/router for medical devices (specialising in connecting to devices and forwarding data). Special interested if they also offer connecting to userfriendly and disguised monitors like iMonsys http://www.telecareaware.com/index.php/innovative-imonsys.html
For service-flats (flats specifically for elderly with basic (paid) services) i'm looking for white-label apps. Besides the registration and interaction (menu-choice, leisure program/subscription , calender planning, ...) such apps should offer medication checks, PERS and video interaction. Here in Antwerp there's a 200-flat experiment in collaboration with Falcom (which runs Linux), but it's too basic imho. Any companies wanting to produce such apps?
Companies that are targeting young elderly people and focus on easy to use location based services, photo-sharing and tourism (food + guidance). If they concentrate on that core service AND also offer basic medical apps/services, they will do well and sell easier. Which companies are doing so?

BUT: I'm not interested at all in mobile phones or services that:

only offer service 'coverage' within a certain range of a specific transmitter/receiver. A lot of fall detection devices are doing this. But older people are very mobile, so their product is useless.
don't have a camera or GPS or Wi-Fi
have small buttons, tiny text, ...
are sold by companies that target OLD people (old people don't think of themselves as old .. and won't buy)

Google Buzz for apps: why is it taking so looooong?

Buzz is an extremely easy and fast way of communicating in your small or large team, even if you're very mobile.

And small to large teams are the target group for Google Apps.

The company buzz can be shielded from the outside world and can be completely integrated with other Google Apps.

Buzz was rolled out early this year and Google premium users are still waiting for Buzz.

Why aren't they implementing it?

There's no decent way to target specific groups of people within your organisation.
Tagging is not possible. And as you can't tag, you can't filter on it or create a filtered RSS-feed ... geared towards specific groups in your organisation.
I'm sure Google will be implementing a modest taskmanager in Buzz for Apps. Might take some time to figure out how they are going to do this.

Whereas Wave was (and is) too difficult for 'regular' users, I'm convinced Buzz has a great future for all Apps users.

I know there are a lot of apps that track your course (runkeeper, nokia, garmin, etc). But these only monitor a limited amount of sports.
Sportypal, Tallyzoo and Endomondo have more disciplines, but are still centred around your personal results.

I want to look at it from a different perspective: "look for exercises (of whatever kind) that where performed in your vicinity"

Interact with people doing the same workout
Challenge people
Search for people doing the same exercise/trajectory
Businesses proposing healthy exercises

Jplan is an online employee scheduler for small to medium sized businesses

This application includes a secured login (user/group-based), management of employee data, rotations per employee, 24/7-planning and scheduling (interim workers and volunteers included), vertical verification, flexible reporting (iReport), controlling fixed and variable payment-options, rollback and easy export to payroll.

Most scheduling software is geared towards larger companies (+100 employees) with a proper personnel department. Small businesses cannot afford a proper personnel manager, let aside complicated software, licenses, training- and installation costs.

The application is written in Java (build to Maven, persistence layer = Hibernate with Spring framework), running on a Tomcat server, coupled to a MySQL database. Backups are done with an Ant-script. Jplan is perfectly scalable and 1 single server can easily host about 100 companies. Hosting costs are relatively small (2.500,00 EUR, incl. hardware) and maintenance-costs minimal.

Jplan has been intensively used for 5 years in a home for the elderly with 30 full time employees. In such a 24/7-environment with irregular services, mainly part time workers, frequently changing planning and flexibility, intelligent reporting and ease of use are an absolute must.

Almost all designing-efforts were geared towards user friendliness and native use. No excessive amounts of buttons, tabs, windows, shortcuts, ... but simplicity as basic principle (less=more). Rapid insights and excellent support for planning and business decisions is guaranteed with the use of web-technology, variables, colors and graphs.

Main advantage when using Jplan is time profit ... during training, while scheduling, when looking up data, when comparing, when making decisions,...
Maximum duration of training is 30 minutes for regular users/planners, 1 hour for administrators. Compared to more advanced spreadsheet solutions, the planner gains about 20 minutes per employee, per month. More important is that efficiency plus intelligent reporting results in very cost effective decisions.
Payroll-firms and their help desks gain time also because, the easier the application, the less questions will be asked, the less staff is needed.

Market potential: When strictly used for homes for the elderly and service flats in Belgium, the potential is about 1.200 organizations (total Belgium = 1.750), these are care-organizations with an employee number between 10 to 70. Two larger Belgian payroll-firms confirmed they could cater about 400 potential customers in this specific category.
However, if this application would be partly re-written, it could be used for no matter which small to medium sized business.

Development time: Start-up took place in September 2003 (writing down specifications, design and choice of open source engines). Version 0.4 was implemented on the work floor in December 2003. From then on the application was continuously developed, together with a professional programmer, until the last update, version 6 in 2008.
Total-spend-time was not stop-watched, but modestly estimated at about 3.000 hours.

Property rights: During meetings and demo's with colleague-entrepreneurs, all where immediately prepared to pay 1.000,00 EUR on a yearly basis, only for the use of this on-line application. I consider this price as an easy-to-use minimal pricesetting.
Due to the simplicity of flexible scheduling employees and the low price setting, a target of 500 Belgian and 700 Dutch firms is realistic.
Jplan saves me about 8 hours administration a month (compared to an elaborate spreadsheet-solution) and the clarifying planning economizes about 25,00 EUR a month, per employee.

Only 98 emails in 2 weeks: Gmail filtering and labeling minihowto

Yep, only 98 emails to respond to after two full weeks of holiday.
Took only 4 hours to handle them all ... inbox back to zero.
These are *cough* important *cough* emails, emails that demand some kind of response from me.

All other emails are filtered away (out of my inbox) and given an appropriate label. These emails are not essential stuff and clutter my inbox. This group of emails consists of newsletters, notifications, updates, news from different social networks, ... after two weeks I had 900+ of them. I should read/handle most of them, but there is no pressure/urgency to do such.
I use Gmail filters and labels to make these -less important emails- skip my inbox and drop them in a labelled "folder".
Your filters/labels could look like this:

Do this: Skip Inbox, Apply label "NEWS"
Do this: Skip Inbox, Apply label "DEV"
Do this: Skip Inbox, Apply label "BIGBOOBIES"

To filter on multiple email addresses you can simply add "OR" between them (my newsletter-filter contains more than 100 email addresses). You can add a wildmark like "@newsletter.com" in your filter to grab all mails coming from a certain source. You can find other search/filter - operators here.
I admit it took quite some time to set this system up and produce the needed filters. I estimate the setup time (from scratch) at about 5 hours.

The filtered setup boosted my email handling efficiency enormously and greatly improved my focus while working. Furthermore I saw a light at the end of the (email-)tunnel ;)
Another advantage is that I consult my email more via the smartphone. As only stuff that matters comes in my inbox, it makes reading/responding much easier.
I estimate the ROIT (return on invested time) at about 1 month.

No rocket science here, just a very simple little triage that saves me A LOT of time.
Do not forget to backup your filters! (labs -> Filter import/export)

KMLCSV Converter is a free open source software that enables you to convert Google Earth KML file to a formatted CSV file (and vice versa). This CSV file can then be uploaded into your Garmin GPS by using Garmin POI Loader, which can be downloaded for free. This allows you to easily plot all custom point of interests (POI) through Google Earth first before transfering them into your Garmin GPS. For further instruction in how to use to do so, please refer to the Tutorial.

Using a Nokia N97 and the new functionality of Posterous, which is autopostr.
This is no 'groundbreaking' technology, but I only want to stress the ease of use. The very simple keyboard (which allows reasonable fast typing) of the N97 and its 5 megapixel camera are the base for this lightweight type of communication. The posterous autopostr updates posterous + blog by sending ONE email. Can't get any easier then that.
Could use a spellcheck though.

I think there's giant market for DIY wireless healthcare monitoring @ home.

Call it a Nokia Sports Tracker framework, with additional health monitoring functions and professional follow up.

People with broadband routers can buy devices (fitness, weighing scale, heart monitor, glucose meters, accelerometers, ...) that interconnect with the router via Wi-Fi.
Companies can offer:

non-expensive devices that are easy to use and install
services/platforms for interpretation of the data (and sharing?)
guidance/advice for end-users
alarm-procedures/interventions

Simplicity = key

Lower communication cost
If your telephony costs are skyhigh, WIFI can surely lower them. Especially in situations with subsidiaries and international communication. VOIP-solutions are evolving rapidly, your organization and your customers will benefit from that evolution.
Wireless care registration
I can't see any reason why you should not switch to VoWIFI if you have already chosen for wireless care registration. Alternatively one could also state that one should not choose for WIFI if you are not going to implement wireless care registration. I would go for wireless registration because of the improved customer service and because of the lower labor time and stress. Scrupulous vendors will subsequently not hesitate to argue that, if you do not choose for WIFI, your organization is not patient-centered or employee-friendly.
Wireless flexibility needed
Lot's of changes in your building, rooms with different functions, flexible workstations, ... lots of cabling continuously needs to get moved?
Hardware flexibility (connectivity) needed
Need to couple WIFI-enabled devices and streaming audio/video? Need the freedom of choice in choosing (different) WIFI-devices from different manufacturers (Nintendo anyone)? Need to trace devices/stock/people (cfr. RFid and such)? Need to couple devices with extensive features like camera, digital pens, bluetooth and iR? WIFI can offer these convenient possibilities and offer a tailor made solution.
Communication flexibility needed
More efficient handling of calls, more possibilities to route communication to email, SMS, web, ... Coupling webtechnology is native. More choice in software solutions. WIFI is not a propriety system like DECT.

Hey, you left out "internet for customers"!
Internet for customers also offers extra headaches for security, QOS, bandwidth, configuration, invoicing and paperwork. And what about concurrent users in central parts of the building, will they encounter the bottle neck on the transmitters? There are too many hidden costs in this service. I suppose that customers/patients who really want to be online continuously already have a wireless subscription of some kind. I don't think this to be an completely pro-argument as this service has many contra's as well.

What about improved team-communication?
Sure, (unified-)messaging, limitless routing possibilities of data, availability of data, design (userfriendly visualizations) will enhance communication and productivity.
But beware of userfriendlyness of the tools and technicality of the applications. In the "ideal" situation the cellphone/PDA should handle: voice, messages, alarms and care registration (I even leave out other functionality). I haven't seen one solution yet that doesn't require employee training. Technicality might be a big cost, technicality might be the cause of faulty registration or interpretation. I would suggest taking some of your staff to a demo and let them respond voice-calls and handle alarms without training or instructions, let them think, observe how they do.

Doubts taken away:

Quality of voice-calls is impeccable.
Handover (going from one transmitter to another) is seamless
Transmission is secure. I checked 3 manuals and all could provide strong security (reminder: stay away from basic WEP or easy WPA-PSK passphrases)
I can't say I experienced interference. But I haven't really tested that item thoroughly. Otherwise I can't imagine that interference could be an issue, because WIFI solutions have been widespread in different offices.
I still haven't got a 100% guarantee that no alarms (like fire) will be lost. But such doubt could be cleared within a SLA.

Calculating WIFI ROI in a healthcare setting

Write down a description of current call- and care-handling in a IF -> THEN way, followed by the needed time to perform that handling.
Write down the ideal handling of call- and care-handling. See what you can improve. Brainstorm with your staff. Write down the findings. Compare times.
Now talk your findings over with two or three WIFI-companies. Check feasibility, ask for further improvement. Make final comparison of times for handling.
Compare hardware costs.
Compare calling costs.

Frustrating in this exercise is that there is no way you can quantify the convenience of flexibility. It is certain that your initial hardware investments will be higher. It is also certain that your future investments and communication costs will be lower (compared to DECT). WIFI-receiving devices are more mainstream, cost is definitely lower.
Uncertain is how much you will actually save because of the future freedom in choosing the best possible solution for your organization.
Clarification: I do NOT want to cut labor time because I want to lay off staff. Cutting time is essential in allowing care-staff to spend more time on the patient/customer ... instead of loosing time with old-fashioned communication. Lowering labor time and stress in healthcare is a major issue. Also frustrating is the fact that it is nearly impossible to compare current and future employee absence caused by stress. You have to rely on your gut feeling here.

Hidden costs

Employee training needed for working with devices.
Employee time lost by devices that are not userfriendly.
Power consumption of the devices (more transmitters + more devices + more battery charging)
WIFI technology is newer, you need more specialized and trained experts. This will result in greater costs

WIFI touchscreen questions
I'm on a quest for a basic wall-mounted touchscreen with identification, browser, onscreen keyboard and WIFI. I already had contacts with Panasonic and Mobilevision but the cost of those devices might be too steep for use in elderly care (about 2.000,00 EUR/piece).
Onboard sound (speakers or headphone in) is a plus, power consumption and absence of moving parts (like ventilation) are very important features.
Such touchscreens could be easy-to-use care-registration and care-consulting devices in homes for the elderly and service-flats. They should be installed in every room, price is essential (read: low). In my humble opinion the screens of domotics, Blackberrys or even PDA's are way too small and do not offer a decent solution for interaction (FTR: SMS on a 12 button phone is not a decent way to interact!).
Maybe the cost could be lowered by a using a fingerprint protected USB-stick with a small operating system on it.

Might also write Nintendo for a more ruggedized version of the Nintendo DS Light for care registration in healthcare.

Manufacturing software solutions resembles more and more to assembly line cooking.

What does it take to produce an easy to use communication platform for team members of a new project, coupled to an extensive document work flow and management application, a bug tracker and time-scheduler, a basic invoicing application, a mobile website for consulting information, embedded video conferencing and chat, LDAP synchronization and with possibilities for customer interaction? The project is limited in time and requires only collaboration during a period of 10 months. The budget is minimal and it should be up and running tomorrow!

Going to your kitchen you get creative with -for example- Alfresco, Scrumy, Task2gather, Assembla, Planzone, ZOHO, OOVOO, Laconica, RTM, Yuuguu, Userplane, Dipity, Getsatisfaction, Joomla, ... tying together the info with plugins or API's or embedding them in a portal solution. Taking a knife to cut away some fat, seasoning the dish with some templates or css and combining different microformats, et voilà: a delicious end-product.

This results is a highly configurable and flexible solution that may grow or shrink during the project ... whatever pleases the hungry client. It will be relatively easy to set up and easy to dismantle afterwards. The solution will not be expensive and perfectly fit the needs. The organization is not forced to invest in extensive courses to cope with this (assembled) product. Team members will not have a steep learning curve. Difficult tools just cut the appetite. The only requisite for end users is the ability to handle a micro-wave-oven or to boil water. There is simply no time and budget for cooking the traditional way.

In this era of focus on product-manufacturing in the long tail, this evolution (or production method) makes it easy to create fresh dishes *exactly* to the clients wishes, it's just a question of combining the wright ingredients in your meal assembly kitchen.

The "possibility of the solution" is not a criteria anymore, other criteria like speed, high touch and feel, userfriendlyness and flexibility come into play. Instant satisfaction is the criterion.

In analogy to TV-shows we may also see rockstar-like software-solution-cooks performing on gigs, producing a surprise assembly-task in front of a crowd, or producing a solution against the clock where the process is displayed on large screens. Show - cooking!

WIFI in healthcare = too many arrows

Never in my life i have underestimated a buyers decision more than the one I am facing right now. This decision is about replacing the current PABX business phone system in a home for the elderly.
As we are building new rooms for new customers, we will have to change the current system as it's maximum capacity will be exceeded.
Replacing the current PABX/DECT solution by a new one seems most obvious. But I am frustrated by the high costs, the limitations and inflexibility towards rapid evolving technology. It is very difficult to add value (through improved team-communication, better response to customers, better logging, better compatibility with third party software, etc.). Buying a telephone solution with PABX/DECT looks like trowing away your money.
Wading through enormous piles of information and brochures, I quickly understood that WIFI gives me the best (future-proof) possibilities and added value. Team communication can be optimized (imagen an interface like MSN/Gtalk/AIM), data and voice is integrated (and coupled) in one system, integration of WLAN and web-based applications, ... flexibility galore, without expensive cabling! Moreover I can improve communication for elderly customers: big button WIFI-cellphones, big screen devices, softphones, message boards (kiosk), free possibilities like Skype, Netmeetings, etc.

The theory is obvious, the practicalities not. I spoke with 9 firms, some once, some 3 times. Each firm contradicting the opinions of their opponents, some firms making mistakes in areas/technologies they are not familiar with, each firm giving more questions and doubts than answers. As a customer I'm forced to study the possibilities and impact of the technology for an objective view.
In this study it's very easy to get completely lost in information overload. As a technical minded user it is very easy to get blinded by the thrill of the chase of new technology.

Bottom line is that WIFI is entangled in so many disciplines within my care organisation that I end up with too much different technologies, too many questions, too many sales reps, too many technicians, ... each defending their technology. I have too many arrows on my diagram ...

WIFI Plus

No slowly dying technology like DECT. "It is clear that VoIP technology will replace traditional circuit-switched telephony in the enterprise. It is no longer a question of if, but when enterprises will migrate to IP-based telephony solutions. This is where WiFi holds a clear advantage over DECT, or any other proprietary TDM-based wireless solution. WiFi allows for end-to-end IP telephony making it the most elegant — and cost-effective — technology for enterprise wireless in the long run."
Connectivity: all WIFI devices can be connected (no propriety installation like DECT)
Compatibility: all (current + future) applications can be coupled to this one voice + data-highway
Limitless configuration- and management- possibilities
Flexibility: end solution being *exactly* what a customer wants, dataflow (like for example UMS) can be tailor made
Possibility to add "tracing" to your network
Cost of communication will go down as more people will use VOIP (instead of landlines)
No expensive wiring.

WIFI Minus

Battery-life of mobile WIFI-devices is 8 hours maximum. In a care-environment this could be insufficient.
Range of WIFI-transmitters is smaller than it's DECT counterpart. You need more transmitters (about 30% more).
Maximum length of an UTP (cat 5) cable is 100 meter.
What if the switch has a defect? Default backup-solution is essential.
Power-out = no communication possible.
Cost of installation (a lot higher than traditional solutions, although no cabling is required)
No completely integrated and proven solution in a healthcare-setting. Where can I personally witness a working WIFI-installation and ask for user-feedback?

Questions

Is quality of voice communication with WIFI inferior to other solutions? As i've used different Java-communication-apps frequently, I can't say I've ever experienced any quality loss though. But this situation is a single user situation.
Is it possible to have different ringtones on a DECT-phone. This is, can the receiver perceive a distinction in ringtones on a mobile dect-device, if it's origin differs. Origin could be: inbound call, room service, fire-alarm, etc.
How much will internal WIFI communication cost? I fail to see any costs, 3 sales reps claim the opposite.
Is seamless handover (from one transmitter to another) guaranteed with WIFI-handsets?
Alarms-handling: How are alarms handled? Is it possible that a certain alarm(-signal) gets lost?
Is WIFI secure? This is the communication itself, access to the network and usage (bandwidth).
How much capacity should the switch have? How many lines are needed for, as example, 10 staff people and 70 customers?
Will WIFI-devices interfere with one another?
How much different actions do i have to perform to make an internal call, to accept an alarm, to put calls through, ... how easy can regular users perform these everyday actions?

Visualising this data would enhance the diagnose

What about have a drug described on a foreign trip and you are unsure about what that drug does and what that strange looking doctor murmured to you in engrish? Take a picture of the drug and upload it to your Flickr-health-account, which displays on the Dipity-timeline. Await your doctors advice. Same goes for that strange rash you got in some tropical forrest.
What about inserting your personal RSS-feed (maybe about your sport activities), conversations with whatever practioner on Twitter, ...
What about diagnosing problems if they occured in a certain location if people's data is geotagged? That could improve upon the speed of treatments for larger groups of people.

Open Source in healthcare
The more you look at it, the more sense it makes to deploy open source software in healthcare. Open source software is customer- and user-centric, it evolves bottom-up. Moreover, open source software can add maximum value, even on relatively short term.

Open Source Software

Wikipedia: "OSS can be defined as computer software for which the human-readable source code is made available under a copyright license (or arrangement such as the public domain) that meets the Open Source Definition. This permits users to use, change, and improve the software, and to redistribute it in modified or unmodified form. It is often developed in a public, collaborative manner. Open source software is the most prominent example of open source development and often compared to user generated content."

Don' make yourselves any illusions, free availability is NOT equal to free! The use of open source software may be free, but adaptation, implementation, installation, configuration of parameters and maintenance are not free. I will omit the calculation of total cost of ownership of open source software, and concentrate on the added value, because this added value unveils the real potential of open source software in healthcare.

Open source software key factors of success

From Open Source Values - Meritocracy, Transparency, and Legitimacy:

Transparency
Open source forces you to stand naked before the world. As the ultimate peer review process, your code is out there for the world to see, touch, and judge. But within the scope of business, it goes beyond just source code. Your business practices are also transparent to the world.
Meritocracy
In an open source world, you are judged by what you DO. You either produce code, close customers, build value, or you die. End of conversation.
Legitimacy
Achieving either of the above, enables you to gain influence with your customers and within your community. And it can’t be a one hit wonder either, as legitimacy implies a proven track record over time. In Open Source, it is all about trusted voices (branding).

Transparency, meritocracy and legitimacy guarantee quality software.

If the source is closed, how can you be sure if the product is safe? If the source is closed, how can you be sure what you buy *exactly*?

Necessities for software in healthcare?

1. User-Friendlyness

Healthworkers should not lose time with long dull boring courses to understand complex software, no more -button overloaded- interfaces, no more restrictions to the number of users, no more (cost-)restrictions regarding adjustability and creativity, no more propriety difficulties regarding integration and migration, no more loss of employee cost and time due to complicated software, ...

2. Interoperability
Software product X should communicate easily with software products Y, Z, F3µ, 15J7, ... Open standards are an absolute must in a chained healthcare system where processes should be tuned in to one another. It is essential that it should be child's play to couple, link, filter, mix, trim, ... different data-streams to one another, internally as externally. User friendly open standards facilitate communication between different user-groups, in the first place between between the directly involved people, namely customers and caregivers. In second place filtered data can be opened up to third parties: MD, family, supplier, pharmacist, logistics, ..., and this by any means (letter, browser, email, SMS, Twitter, ...). Interoperability gives the possibility to rearrange data from a variety of different data-streams or mashups, in such a way that the end-result enables quality decision-making. In healthcare decisions should not be hindered for propriety reasons.

3. Customisation and Simplicity
This third factor comes forth from the first two. Open source allows easy altering of software in accordance with the demands and wishes of the end-users. If one is allowed to alter layout, parameters and connections, it is quite obvious that the final product will be tailor made and trimmed down to it's beautiful simple essence.

4. Speed
Development velocity of open source is very high. Main cause is the large number of developers that is actively involved worldwide. Chances are fairly high that someone, somewhere is working on a similar solution or connection, and will be happy to share his or hers knowledge on the subject. Open source encourages creativity and the development of all kinds of extra functionalities (add-ons, widgets, modules, ...) and connections. This results in extreme flexibility; not only in swift reactions to rapid changing legislations, but also in coping with fast evolving customer expectations (paradigm shifts).

From Open Innovation (Henry Chesbrough): Over time, organizations that learn faster will outperform even quite capable organizations, if those latter organizations are slow to adapt to a changing environment. As business environments become more complex and more turbulent, it becomes increasingly important to be adaptive. Playing poker becomes more and more useful, whereas playing chess in such environments is less and less effective.

Efficient, interconnected and userfriendly software tools allow meaningful and swift registration of care, avoiding mistakes and assure an improved treatment. Moreover they lower labour time, cost and stress. Probably they even heighten employee job satisfaction and lower absence rates!

Management
Efficient, userfriendly and open products simplify data-coupling, data-filtering and data-combination and thus allowing to build intelligent cockpits. This enhances and simplifies decision-making in healthcare, on whatever level in the organisation.

Governement / policy
Better managed healthcare = less costly healthcare. Transparency leads to significant cost-reductions by eliminating double-work and confusion, improvement of cooperation between participators and avoiding errors which could lead to dangerous situations.

Open systems and standards are the foundations of quality decision-making
Open communication improves internal and external communication, without restrictions to numbers of users and without restrictions to connectivity (interoperability) with third-parties.
Open source exceeds other approaches in user-friendlyness: less bloatware, more flexibility, rapid development cycles, an abundance of techniques to make the end product look aesthetically pleasing, no restrictions towards creativity or towards innovations and completely cross-platform.
Open source gives us the opportunity to create added value by offering new functionalities and interconnectivity.
Open source enhances healthcare services by improved decision-making and customer interaction.

Other developments seem egocentric and less capable of producing added value, not even on relatively short term.

Who wants to be hostage of expensive licenses, slow development, closed standards, bloatware, inconnectivity and incompatibility? Healthcare clients do not ask for high prices and obstructed decision-making, solely caused by market-protection or greed or loud mouths.
